    THE NAVIGATOR IN COLLABORATION WITH OTHER PHH SERVICES PROVIDES AND COORDINATES ALL ASPECTS OF TREATMENT, EVALUATION, SUPPORT SERVICES AND EDUCATION FOR WOMEN RECEIVING SERVICE ACROSS THE PHH BREAST PROGRAM. PROVIDES CLINICAL FUNCTIONSAND COMMUNITY OUTREACH AS REQUIRED IN SUPPORT OF BREAST CENTER SERVICES. PARTICIPATES IN MAINTAINING A SAFE, COST-EFFECTIVE PATIENT-CARE ENVIRONMENT.

    Responsibilities

    *Complies with PENN HIGHLANDS DUBOIS policies and procedures, accreditation agency requirements, and federal, state or local law and regulations

     
    Develops and manages the care pathway for breast service patients.


    Collaborates in the development and monitoring of a measurement system which ensures that services are performing within defined standards.


    Provides holistic care, education, support and advocacy to the patient & her family.


    Identifies and develops resources to meet the educational and psychosocial needs of breast center patients.


    Demonstrates the ability to articulate the breast programs philosophy in a positive manner to health care providers and the public.


    Serves on committees of the Breast Center and as a consistent contact for patient/family throughout the process of diagnosis and treatment.


    Maintains two way communications with physician offices in order to improve the process of coordinating care for breast center patients.


    Maintains a positive and collaborative relationship with medical staff leadership of Breast programs such as breast radiologist and surgeons.

     


    Reports outcome of nursing/patient care and coordination activities to program leadership.
    Provides education in Breast Self  Awareness as appropriate.


    Demonstrates performance improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational Systems/ Safety.


    Coordinates patient care with other treatment team members. 


    Provide education to professional staff through consultation and didactic presentations.


    Evaluate patient, nursing and community educational programs and materials.


    Provide opportunities for informal education for patients, families & professional staff.


    Demonstrate in depth knowledge of breast cancer risk factors, diagnoses, disease processes, and treatment options.


    Other duties as dictated

     

     

    Qualifications

    • CURRENT PENNSYLVANIA REGISTERED NURSE LICENSE REQUIRED, BACHELOR DEGREE IN NURSING PREFERRED
    • HEALTHCARE PROVIDER (CPR) CERTIFICATION REQUIRED
    • ACLS CERTIFICATION REQUIRED.
    • EXCELLENT REASONING ABILITY AND GOOD COMMUNICATION SKILLS
    • ONCOLOGY EXPERIENCE PREFERRED
    • EDUCATIONAL PROGRAM DEVELOPMENT AND PRESENTATION SKILLS PREFERRED
