    POSITION SUMMARY:  The Intake Specialist facilitates incoming referrals from outside agencies into Penn Hghlands DuBois Inpatient Behavioral Health Units along with facilitation of tele-face to face assessments from Clearfield, Elk and Brookville Emergency Departments.


    *Complies with PENN HIGHLANDS DUBOIS policies and procedures, accreditation agency requirements, and federal, state or local law and regulations


    Demonstrates knowledge of age-related needs of the patient; utilizes the biopsychosocial assessment process on a continuous basis in the delivery of care as evidenced by:

    • Obtains health and social information through interview process, assessment, and data collection
    • Collaborates with multidisciplinary health care team regarding assessment findings and plan of care
    • Identifies patient needs and problems through assessment, interview, and observation including information from significant others and referral sources
    • Implements plan of care approved by with multidisciplinary team
    • Performs referral of care needs assessment throughout the medical center as directed

    Demonstrates judgment and clinical competency in the implementation of the treatment plan for assigned patients as evidenced by:

    • Utilizes clinical and social resources
    • Provides supportive therapies as assigned
    • Responds appropriately to changes in patient condition
    • Reviews medical record to obtain needed patient information

    Participates in the admission, treatment, and discharge process with the multidisciplinary team as evidenced by:

    • Identifies patient biopsychosocial and learning needs on admission
    • Collects information from appropriate outside resources

    Demonstrates judgment and clinical competency in the facilitation of referrals, including tele-face to face assessments and third party referrals as evidenced by:

    • Communicates with all necessary parties to assure efficient flow of patients to the respective unit
    • Ensure that all necessary clinical information is obtained, documented, and communicated to the appropriate staff of the respective unit potentially receiving the patient
    • Continuously gather and process census information via the Teletracking Bed Placement Capacity System to assure efficient flow between units and that unit capacity is maximized.
    • Complete referral data collection and analysis

    Participates in the development of new staff as evidenced by but not limited to:

    • Actively participates in the orientation including cultural and social integration of new staff
    • When serving as preceptor, collaborates with staff member and manager/charge nurse to manage the learning process and develop individualized action plans as needs are identified

    Demonstrates professional accountability as evidenced by but not limited to:

    • Achieves educational credits to meet hospital and/or licensing/certification requirements
    • Presents information obtained at education conferences to staff members
    • Participates in the development and advancement of departmental standards, policies, and other clinical/professional initiatives
    • Conducts self in professional manner in dress, actions, and interpersonal communication
    • Ensures environment of safety for patients, significant others, and coworkers
    • Informs Director/Supervisor/Clinical Manager of patient care situations and departmental problems as appropriate
    • Demonstrates effective problem solving skills for clinical and departmental issues

    Ensures an environment of patient safety through:

    • Performs patient safety/observation checks every 15 minutes
    • Complies with contraband policy
    • Manages changes in behavior that puts the patient or others at risk
    • Complies with restraint/seclusion policies

    Demonstrates and maintains ongoing competencies for:

    • Facilitates an efficient and timely review and disposition of referrals consistent with department policy

    Maintains current CPR and Comprehensive Crisis Management certifications


    General duties as directed



    • Bachelor Degree in Social Work, Counseling, Psychology or related field or Bachelor Degree plus two years behavioral Health experience required
    • Previous behavioral health experience preferred
    • Healthcare Provider (CPR) certification required within 6 months of start date in department/position
    • Comprehensive Crisis Management certification required within 12 months of start date in department/position

    **Will work every other weekend

    **Will work 3 holidays per year
