The primary job responsibility of the Transfer Center ALSA is to receive and coordinate calls regarding acute patient transfers, consults, and/or admissions from medical facilities. The ALSA utilizes patient logistics software to coordinate patient flow and bed placement. The ALSA speaks directly to the referring and receiving providers/staff to coordinate patient care. Strong communication skills and knowledge of medical conditions and terminology is required. When not managing calls the patient transfer center, the ALSA participates in the care of Emergency Department patients. Clinical responsibilities to include (but not limited to) the transport of monitored and non-monitored patients to inpatient units and diagnostic areas, obtaining patient's vital signs, EKGs, IV initiation, phlebotomy, assisting physicians with beside procedures, and participation in the care of advanced life support cases.
Complies with PENN HIGHLANDS DUBOIS policies and procedures, accreditation agency requirements, and federal, state or local law and regulations.
Demonstrates competency in Patient Transfer ALSA role as evidenced by but not limited to:
*Receives physician requests from transferring facilities and coordinates patient transfer to receiving facility.
*Professionally and accurately represents Penn Highlands services.
*Develops and maintains referral relationships with outlying facilities through consistent customer service and ease of use.
*Facilitates all communications between referring and receiving physicians and other hospital staff.
*Documents all required information pertaining to the patient transfer process.
*Accurately enters data into the database as it occurs for creation of real time data.
*Coordinates patient flow and logistics within the Penn Highlands Emergency departments and inpatient units.
*Contributes information to a registered nurse regarding the assessment of the health status of emergency department patients.
*Transports monitored and non-monitored patients to inpatient units and diagnostics.
*Obtains vitals, interprets cardiac monitors, initiates/discontinues IV fluids, assists physicians with bedside procedures.
Participates in the development of new staff as evidenced by but not limited to:
*Actively participates in the orientation including cultural and social integration of new staff.
