THE DIETITIAN DIABETES EDUCATOR IS RESPONSIBLE FOR IDENTIFYING THE NUTRITIONAL NEEDS OF PATIENTS AND DEVELOPING A PLAN OF INTERVENTION TO IMPROVE THE PATIENT’S NUTRITIONAL STATUS, AND EDUCATING PATIENTS CONCERNING THERAPEUTIC DIETS. THE DIETITIAN DIABETES EDUCATOR IS RESPONSIBLE FOR ASSESSMENT AND EVALUATION OF THE DIABETES PATIENT NEEDS AND FORMULATES A RESPECTIVE CARE PLAN. EDUCATES PATIENTS WITH DIABETES, BOTH INPATIENT AND OUTPATIENT ON SELF-MANAGEMENT TECHNIQUES. PROVIDES UP-TO-DATE INFORMATION ON DIABETES EDUCATION TO ENTIRE PATIENT SERVICES STAFF

Assesses and evaluates patient needs and formulates a nutrition plan of care which includes long-term and short-term objectives, nutritional requirements, education needs, on-going monitoring for changes/new information and evaluation of outcomes.
Plans, organizes, implements and evaluates nutrition education for clients both individually and in group sessions; arranges for individual follow-up as needed.
Instructs inpatients/outpatients on self-management /assessment pertaining to diabetes, symptoms, treatments and effects.
Properly documents assessment data, educational plan, educational services provided and evaluation results in each patient’s medical record.
Assists the Physician by instructing and managing patients starting insulin therapy via syringe, insulin pump or insulin pen
Assists the physician by instructing and initiating patient’s use of continuous glucose monitoring (iPro) and downloading reports.
Participates in educational and community involvement opportunities.
Assists the manager with ongoing reviews and quality outcomes pertaining to educational processes
General duties as directed.
