The Physical Therapist is responsible for providing direct patient care through evaluation and treatment of patient's injuries and illness.

Responsibilities

  •  Complies with DRMC polices and procedures, with accreditation agency requirements, federal, state, or local law and regulations.
  • Performs patient evaluation, including all pertinent areas based on diagnosis and history
  • Establishes short-term and long-term treatment goals based on evaluation, patient and significant others
  • Plans and implements age appropriate treatment program, taking precaution to observe any contradictions.  Modifies treatment program as indicated
  • Maintains necessary records including, but not limited to initial evaluation, weekly progress notes, discharge summary, treatment record, functional independence measure and physician certification forms
  • Attends and reports at team conferences involving your patients.  Confers with other health professionals, as needed.  Plans and instructs patient and significant other in home programs.
  • Serves as clinical instructor for physical therapy and physical therapy assistant students (when meeting requirements)
  • Supervises the treatment given by assistants and technicians

Coordinates and implements patient teaching and discharge planning sessions for assigned patients and significant others as evidenced by:

  • Initiates discharge planning and collaborates with other health care team members to achieve goals
  • Develops and implements a teaching plan for discharge
  • Evaluates the results of patient teaching and discharge planning
  • General duties as dictated

Qualifications

  • Licensed Physical Therapist in Pennsylvania required.
  • Experience working as a physical therapist preferred.
  • Successful completion of FIMS within six months of start date if working Rehabilitation Center required.
