The Home Health Care Aide, under the supervision of professional nursing personnel, assists and provides patient care utilizing standards of care.

Responsibilities

*Complies with DRMC policies and procedures,  accreditation agency requirements, and federal, state, and local law and regulations

*Demonstrates knowledge of age-related needs of the patient, provides patient care based on these needs as evidenced by but not limited to:

  • Assists with toileting, bathing, foley care, ambulation, feeding, mouth care and positioning of patients
  • Obtains vital signs, intake and output, and weights per patient request
  • Performs procedures under direct supervision of the RN as deemed competent (i.e. dry dressings to unbroken skin)
  • Responds promptly to requests and meets patient care needs or reports to the Registered Nurse as appropriate
  • Maintains cleanliness of clean and dirty utility rooms and patient care areas
  • Restocks IV trays and supplies in medication carts daily
  • Maintains order within the equipment storage areas
  • Provides patient care based on the priority of needs and available resources
  • Reports changes in patient's condition to Registered Nurse and follows-up
  • Participates in patient case conference
  • Utilizes cost effective measures in delivering patient care
  • Monitors the health care environment and implements measures to maintain safety of staff, patients, and home environment Provides and/or assists with personal care as established by a Home Health staff nurse or appropriate therapist.  This care is specified in the Aide Care Plan
  • Assists patient with prescribed exercises that the patient and Home Health HCA has been taught by appropriate professional personnel
  • Completes daily charges
  • Documents care and treatment on clinical record daily
  • Supports and demonstrates understanding of mission and goals of the unit
  • Identifies patient problems/focus through observation and interaction
  • Implements patient care in accordance with DRMC standards of care, policies and procedures

*Participates in the development of new staff as evidenced by but not limited to:

  • Actively participates in the orientation including cultural and social integration of new staff

*Demonstrates personal accountability as evidenced by but not limited to:

  • Achieves educational credits to meet hospital requirements
  • Participates in the improvement of department through individual or team contributions
  • Conducts self in professional manner in dress, actions, and interpersonal communication
  • Assures environment of safety for patients, significant others, and coworkers
  • Informs Director/Manager/Supervisor of patient care situations and departmental problems as appropriate
  • Demonstrates effective problem solving skills for clinical and departmental issues

Qualifications

  • Customer service attitude required
  • High school diploma or equivalent required
  • Current Pennsylvania Health Care Assistant (CNA) certification required
  • Familiarity with medical terminology required
  • Clinical experience as nursing assistant within the previous two years of hire required
  • Current CPR certification required
  • Current Pennsylvania driver’s license and automobile insurance required
  • Private vehicle for transportation during assigned working hours required
